How Does Sclerotherapy Work?

Spider veins are threadlike veins that are visible because they are too close to the surface of your skin. Sometimes these veins rise as a result of injury or from years spent sitting at a desk. Sclerotherapy is recommended by many dermatologists for the treatment of varicose and spider veins.

The treatment follows these steps:

  • Sclerotherapy irritates the problem veins, causing damage.
  • The damaged veins close.
  • Eventually, the vein scars beneath your skin.
  • Once the vein scars, it disappears.

What is Sclerotherapy?

Sclerotherapy is an injection technique used to get rid of primarily leg veins. Sclerotherapy is a popular method to clear veins primarily of the legs by injecting sclerosing agents directly into the spider veins. No anesthetic is required. A strong solution (the sclerosant) is injected directly into the blood vessel causing inflammation of the walls of the vessel. The vessel usually disappears over a few weeks to months.

How Does Sclerotherapy Work?

Sclerotherapy involves an injection of a solution (generally a salt solution) directly into the vein. The solution irritates the lining of the blood vessel, causing it to swell and stick together, and the blood to clot. Over time, the vessel turns into scar tissue that fades from view.

How is the procedure performed?

Sclerotherapy is performed with the injection of a salt solution through a very fine needle directly into the vein. You may experience mild discomfort and cramping for one to two minutes, especially when larger veins are injected. The procedure itself takes approximately 15 to 30 minutes.

What should I avoid before the procedure?

It is recommended avoiding aspirin, ibuprofen (such as Advil, Motrin, and Nuprin) or other anti-inflammatory drugs for 48-72 hours before sclerotherapy

What are the aftercare instructions following Sclerotherapy?

Cotton wool pads under adhesive plasters are applied to the injection sites. Pressure is applied to the treated vessels with compression hosiery (support stockings) &/or bandages. These are usually worn for between seven days and three weeks following each treatment.

Patients are usually able to return to normal daytime activities immediately after sclerotherapy. They should walk for at least thirty minutes a day, especially for the first few days following the procedure. Vigorous physical activities such as weight lifting or aerobics classes are not advised during this time.

What complications can occur from the procedure?

In most cases, the pain, discomfort, and swelling associated with the veins disappear within days, and the vessels slowly resolve over several months. Milder effects, such as itching, can last for one or two days after the procedure. Also, you may experience raised, red areas at the injection site. These should disappear within a few days. Serious complications following sclerotherapy are rare. More frequent complications include the following:

  • Staining or brown pigmentation, at the site or along the line of the vein. This occurs in about 30% of patients. In most cases, this resolves but it may take many months.
  • Clots within treated vessels. These are not dangerous but can be quite tender if they occur in the larger veins. They can be removed through a needle prick after several weeks if necessary, but left alone they will eventually be reabsorbed.
  • Temporary bruising. Bruises at the injection sites are quite common but resolve within a week or so.
  • Capillary matting. This is an increase in the number of fine red vessels around the injection site: These tiny veins can appear days or weeks after the procedure but should fade within three to twelve months without further treatment.
